We are looking forward to seeing you for your examination with our urologist.

To help with the evaluation of your condition, we ask that you complete the forms available on our "Forms" page, linked on the menu above. If this is a yearly exam, these forms are required to update your records annually. If you are a new patient to our office, they are necessary to establish your history and current physicial condition.

Please bring your insurance card(s) and photo ID; we will need a copy for your chart. We also ask that you be prepared to give a urine specimen in our office, upon registration. If you have any recent x-rays, please bring them to your appointment. You will need to arrive 15 minutes before your appointment time.

We have converted all of our patients to Electronic Medical Records to better serve you. In order to make your visit as effective as possible, we ask that you complete the patient forms and return them at least 72 hours prior to your appointment. Failure to do so could result in your appointment being delayed or rescheduled.

Charges for medical services at our office are due and payable at the time services are rendered. Our physicians do participate with Medicare. If you have Medicare, you or your insurance will be responsible for the 20% balance. HMO patients should be prepared to pay your co-pay.
If you are uninsured, please contact our billing department at (231) 739-9492, option #2, prior to your appointment.


If you cannot keep your appointment, we request 24-hour advance notice to cancel. This makes it possible to give the appointment to another patient.
